Portfolio Assessment Form

The purpose of this form is to develop a portfolio whereby credits may be given for life learning experience. Bethany allows you to receive up to 32 credits for learning experience. However, these credits can only be applied in the undergraduate level. Also, it allows the college to do an evaluation of where you would be placed should you decide to enroll.
